OET vs IELTS for AHPRA: Which English Test Should Nurses Choose in 2026?

What AHPRA actually requires

AHPRA accepts two English language tests for internationally qualified nurses: IELTS Academic and OET (Occupational English Test). The required standard is identical for both: you must achieve the equivalent of IELTS 7.0 in every individual component.

For IELTS Academic, that means 7.0 in each of the four bands — Listening, Reading, Writing, and Speaking — with no exceptions for overall score. A 7.5 average with a 6.5 in Writing is a fail.

For OET, the required standard is Grade B in each component — Listening, Reading, Writing, and Speaking. An important update from 2025: AHPRA now accepts Grade C+ for the Writing component, which had previously required a full Grade B. This change makes OET Writing more achievable and is the single most significant update to AHPRA's English requirements in recent years.

One test result covers both your AHPRA application and your ANMAC skills assessment — you do not need to sit two different tests for the two processes. Make sure your result is still within its validity period when both applications are assessed. IELTS results are valid for two years from the test date. OET results have varying validity periods depending on the recognising organisation — check AHPRA's current requirements before booking.

IELTS Academic — what it involves

IELTS Academic is a general-purpose English proficiency test used across universities, migration programmes, and professional registration bodies worldwide. The Academic version is mandatory for AHPRA — IELTS General Training is not accepted.

The test has four components: Listening (30 minutes), Reading (60 minutes), Writing (60 minutes), and Speaking (11–14 minutes). The Writing section requires two tasks: summarising a graph or diagram (Task 1) and writing an argumentative essay on a general academic topic (Task 2). Task 2 carries more weight in the final band score.

The cost varies by test centre but is approximately AUD $395 in the Philippines and similar in the UK and Ireland. Computer-based IELTS (CBT) returns results in three to five days; paper-based takes up to 13 days. There are frequent test dates throughout the year at centres across the Philippines, including Manila, Cebu, Davao, Iloilo, Cagayan de Oro, and Baguio.

Results are valid for two years. If you do not achieve the required score in one attempt, you can resit any or all components without waiting — there is no mandatory gap between sittings.

OET — what it involves

OET (Occupational English Test) is a healthcare-specific English proficiency test. Unlike IELTS, every task uses medical and clinical scenarios. The Listening component plays healthcare consultations. The Reading section uses clinical texts and patient information. The Writing task requires a referral letter or discharge summary — real documents that nurses actually write. The Speaking component is a patient role-play with a trained interlocutor.

OET is available in profession-specific versions. Nurses sit OET — Nursing, which uses nursing-specific scenarios throughout.

The cost is approximately AUD $587 per attempt — roughly AUD $190 more than IELTS. Test dates are less frequent than IELTS, and available centres in the Philippines are limited to Manila and Cebu. Results are returned within 14–17 business days.

The 2025 Writing update is significant: the Writing component now requires Grade C+ rather than Grade B, aligning with the standard used by other UK registration bodies. For most nurses, this means Writing is now the most achievable OET component rather than the hardest.

Head-to-head comparison

The honest answer is that neither test is universally easier. The right choice depends on your clinical English strengths and weaknesses.

OET suits nurses who are confident in clinical communication — patient interaction, clinical documentation, handovers — but find academic writing difficult. The referral letter writing task is far closer to what nurses actually do than IELTS's argumentative essay. Nurses who struggle with IELTS Writing Task 2 consistently report that OET Writing feels more natural.

IELTS suits nurses with strong general academic English — those who have recently studied in English, have experience with essay writing, or have failed OET on a specific component and want a different format. IELTS is also cheaper, faster, and has more available test dates, which matters if your timeline is tight.

The pass rate data is instructive. Across Filipino nurse cohorts, IELTS Writing 7.0 is the most common single failure point — many candidates achieve 7.0+ in Listening, Reading, and Speaking but fall short in Writing. OET Writing, now at C+, is more achievable for clinical writers. If Writing is your weak band, OET is the stronger strategic choice.

Which test for which nurse

For Filipino nurses with PRC registration whose experience is entirely in the Philippines, OET is generally the better choice. Clinical English in Filipino hospitals is strong; academic argumentative writing is not a daily skill. The OET scenarios — patient role-plays, referral letters — map directly to ward experience.

For UK or Irish nurses applying via the streamlined pathway, the English requirement is waived entirely if you hold a current NMC or NMBI registration — you do not need to sit either test. This is one of the most significant advantages of the streamlined pathway and is often overlooked.

For nurses who have already attempted IELTS and failed on Writing specifically, switching to OET is worth serious consideration given the 2025 C+ Writing update.

For nurses prioritising cost and speed over content match, IELTS remains the faster and cheaper option, with more available test dates.

The practical recommendation: take a full practice test for both before committing. Most IELTS and OET preparation providers offer free practice materials. Your weakest area will tell you which test to sit.

Preparation timeline

Allow a minimum of eight weeks of structured preparation for whichever test you choose. Twelve weeks is more realistic if English is not your daily working language or if you have a specific weak component.

For IELTS: focus Writing Task 2 preparation from week one. This is the hardest component to improve quickly and the most common failure point. Practice timed essays on general academic topics, get feedback on band-score level, and identify whether vocabulary range or argument structure is your weakness.

For OET: practice referral letter writing from week one using the OET sample tasks. The letter must be formatted correctly — inappropriate tone, missing clinical details, or incorrect structure can drop you below C+ regardless of language level. OET's official preparation materials are the most reliable source.

Both tests require consistent Speaking practice. Filipino nurses often score well on clinical knowledge questions but lose marks on fluency, pronunciation, and response length. Record yourself and compare to band descriptors.
